The Health Coach, LLC is a senior advocacy and care management company dedicated to helping older adults, individuals with disabilities, and their families' overcome obstacles and find the support they need.

We provide a full range of services, including assessments, care planning, care coordination, ongoing care management, patient advocacy, crisis intervention, counseling, transitional care, family/caregiver education, long-range planning, financial organization and bill pay assistance, and more.
