Oftentimes, unexpected medical changes cause our lives to be more difficult to manage than they once were and without the proper supports in place health can decline faster due to stress.
The Health Coach, LLC is a Care Management senior advocacy company that is dedicated to helping seniors, families and individual with disabilities to overcome obstacles.
The Health Coach, LLC provides care coordination, care management, counseling, transitional care, crisis management, patient advocacy, assessments, guidance, care giver support/education, strategies for long range planning, money management/bill pay, and much more.
As care coordinators, we are the aging life care experts in the field and the central linkage that assesses needs and addresses those needs by putting supports and services in place to achieve your goals. Care Coordination helps to communicate among all medical providers and ensure all health goals are being addressed properly for optimal health outcomes.
As care managers we provide ongoing support and monitoring. We keep care needs and services synchronized so that needs are met and not falling through the cracks. Every client is unique with different needs, so we tailor our care plans to help you achieve your goals to live your best life.
The Health Coach, LLC is dedicated to providing the highest level of quality care, guidance and compassion to our clients. We work hand-in-hand with you to better navigate the difficulties of aging, disability, chronic health conditions, family role changes, and various other life troubles.
